In May of 2019 the Northshore School District began a transition away from Family ID and started using Final Forms. You can learn a bit more about this here.

You will need the following information to allow for accurate completion of your online registration:

• Basic Medical History & Health Information
• Insurance Company & Policy Number
• Doctor & Dentist Contact Information
• Hospital Preference
   

Creating an Account

FinalForms sends an email to parents and students with steps to create a FinalForms account. Emails are only sent if the is in grades 6-12. The student must be enrolled in the Northshore School District before an account creation email is sent. If you need assistance creating your account, please contact FinalForms technical support.
 

Registering a Student

1. Visit https://northshore-wa.finalforms.com/
2. Click LOGIN under the Parent icon.
3. Click INCOMPLETE FORMS.
4. Select the sports for which you would like to register. Your selection can be changed at any time prior to the registration deadline.
5. Complete each form and sign your full name in the Parent Signature field at the bottom of the page. After signing a form, click SUBMIT FORM and move on to the next form.
6. You will see  a “Forms Finished” message when all forms are complete.
7. If required, an email will automatically be sent to the your student, prompting them to complete the forms requiring their signature.
8. Click MY STUDENTS when you are done. Repeat steps 3 - 6 for additional students.
9. If you need to update your information, click the UPDATE FORMS button.

Once a student is registered it is very important that they take steps to maintain their athletic eligibility throughout the sports season. While not all inclusive, here are some very important eligibility requirements to keep in mind:

• Have a current physical on file with the main office.
  • Physicals are valid for two years from the exam date and must be valid through the entire season.
  • It is the student's responsibility to check on the expiration date.
• Students must maintain a GPA of 2.0 and be passing at five of six classes.
• Pay all required and any outstanding fees/fines. (ASB membership, sports participation fee, etc.) Students will not be allowed to participate in games, meets or matches until ALL fees are paid.
• Students must attend two of three academic periods on a block day or four of six academic periods on a six-period day to be eligible for that day’s game/event.

Participation Fees

All students participating in a middle school sport must purchase an ASB card for $30. These funds directly support the year-to-year cost of the athletic programs.

There is a $200 fee for each sport. Individuals participating in more than one season will pay a maximum of $400 per school year.

Families with two or more students participating during the same season will be charged $150 per participant up to a family limit of $600 per school year.

Families with two or more students participating at both high school and middle school level will have a family limit of $700.

Fees do not guarantee a roster spot or playing time. Students from low-income families may qualify for a fee waiver. Students who need assistance should contact their school counselor, administrator, or coach for information.

Payments

Please note that payments cannot be accepted until the second week of a sports season. This is done to allow cut sports time to finalize rosters and to bring middle and high school policies into alignment.

Once payment options have opened for a season they can be made in person at the school office or on schoolsales.nsd.org.
 

Refunds

Here is an overview of refund polices:

• If a student is injured, has an illness that ends participation or withdraws from school before the mid-point of a season, a full refund will be granted if requested.
• If more than 50% of the season is completed, a 50% refund will be granted.
• Any student who quits a team will not be eligible for a refund.
• Students who prepay and are cut from a team will be issued a full refund.

Student Accident and Health Insurance

The health and safety of our students is one of the district's foremost concerns. Even so, accidents may and do happen and the resulting medical bills can be very expensive.

• Many families do not realize that school districts do not carry insurance for student personal injury costs and cannot assume responsibility for them.
• If your child gets hurt during school activities you, as the parent/guardian, are responsible for any medical bills. This includes sports activities.

As a service, we are providing this information regarding optional student accident and health insurance coverage administered by Myers-Stevens & Toohey, Inc.

Coverage is available for:

• Interscholastic Tackle Football 
• School-Time Accident Plan
• Full-Time 24/7 Accident Plan
• Accident & Sickness Plan
• Dental Accident Plan

These low cost plans are used by many school districts across the state. Although they are not the only provider of this type of coverage, they have been recommended by our Washington Schools Risk Management Pool.

Filing a Claim

Report school related injuries to the school immediately. Complete the claim form found on the Myers-Stevens & Toohey website. The school will need to complete Part A of this form. 

• Physical Examination Form (English)
  The Physical Examination Form is required for all student wishing to participate in middle school athletics.
   

• Physical Examination Form (Spanish)

Students are required to complete a WIAA contract if they do not attend the school full-time.
 

Off-Campus Classes

If you are enrolled in any off-campus classes, complete the Alternative Education and WIAA Eligibility contract, print your class schedule, and return to the athletic office.
 

Home School

If you are home schooled, complete the Home Based Education and WIAA Eligibility contract, print your class schedule, and return to the athletic office.
 

Grades

Grade checks are run throughout each sports season. Please provide a copy of previous semester and most recent quarter grades. Failure to provide grades may make you ineligible for competition.

If you attend any other school for ANY classes, you must provide the office with your grades from that school.
